Staff COVID-19 Reporting Form

If you are experiencing coronavirus symptoms, living with someone who is either experiencing symptoms or has tested positive for coronavirus, or you have been contacted by NHS Track and Trace you must complete this form to inform the College.

If you have any of the main symptoms of coronavirus:

- Get a test to check if you have coronavirus as soon as possible.
- You and anyone you live with should stay at home and not have visitors until you get your test result – only leave your home to have a test.

if this form is not completed fully or is inaccurate it may affect the return to College date.

Data protection and confidentiality

Where necessary East Sussex College will share this information with NHS Test and Trace in order to prevent and manage outbreaks of Covid-19.

Collection of information to prevent and manage outbreaks of COVID-19
  • Students and staff will be asked to provide the College with information if they have Covid-19 symptoms or a positive test result. This information will be used to prevent and manage outbreaks of Covid-19 and to ensure that staff, students and the community are protected from the impact of Covid-19.
  • Students and staff will be asked to provide information on:
    • Whether they have had a Covid-19 test and if so what the results were
    • Where in the last 48 hours they been on College campus(es)
The legal basis for processing this data

This information will be processed under the Data Protection Act 2018 and the General Data Protection Regulation. The lawful grounds for processing the personal data are:

  • Processing is necessary for compliance with a legal obligation to which East Sussex College is subject
  • Processing is necessary for the performance of a task carried out in the public interest
  • The processing of special category data is necessary for the purposes of performing or exercising obligations or rights which are imposed or conferred by law in connection with employment, social security or social protection and in line with East Sussex College's Covid-19 Outbreak Prevention and Control Plan.
